PVAC is partnering with Commonwealth Care Alliance Senior Care Options to provide asthma home visits for elders with asthma. This program is for adults 65 or older, with a hospitalization or ER visit for asthma in the last year. They must also be enrolled in Commonwealth Care Alliance Senior Care Options.
